Important Note — Minimum Lead Times For Booking:

  • Virtual Sessions (Individual): Please select a start date at least 5 business days ahead from today
  • Virtual Sessions (Group): Please select a start date at least 10 business days ahead from today
  • In-Person Sessions (Group, up to 12 participants): Please select a start date at least 14 business days ahead from today
  • In-Person Sessions (Group, above 12 participants): Please select a start date at least 21 business days ahead from today

Bookings that do not meet the minimum lead time may not be accommodated. WCH will contact you to reschedule if required.

  • Special Requirements or Notes: Accessibility needs, language preferences, specific concerns, or other relevant information for your practitioner

What You'll Need To Provide

Please provide a primary contact person for all booking communications, along with individual details for each participant.

Primary Contact (SPOC):

  • Full Name: Primary contact person
  • Contact Details: Phone number, email address, and emergency contact

Your primary contact will receive all booking confirmations, updates, and program communications.

Participant Details:

  • Total Number of Participants: Enter the headcount (e.g., 1–10)
  • Participant 1: Required for all bookings
  • Participants 2–4: Required for group bookings with more than 1 participant
  • Participants 5–12: Enter 'NA' if not applicable

⚠ Bookings Above 12 Participants:

If your group exceeds 12 participants, you may submit the entire participant list via email instead of filling in the individual fields above. Send your complete list to wellnesscirclehub.co@gmail.com using the required format above.

For all 12 participant fields in the form, simply enter: Submission via email

Please include your booking reference number in the email subject line. WCH will compile all participant details before confirming your session.

⚠ Making A Group Or Bulk Booking?

Before completing your acknowledgments, please ensure the total quantity selected in your order accurately reflects the number of participants for this program.

  • For services / programs: Total quantity = number of participants attending

Incorrect participant counts may affect pricing, practitioner assignment, facility arrangements, and logistics coordination.

Please review and acknowledge all terms before your booking is confirmed. Your consent is required to proceed.

1. WCH Logistics & Delivery Policy
Acknowledge the terms governing program delivery, zone-based surcharges, facility bookings, and travel logistics.

2. Cancellation & Rescheduling Policy
No cancellations permitted. Rescheduling requires at least 7 working days' notice and is subject to approval. All travel and facility surcharges are non-refundable.

3. Informed Consent for Professional Services
Consent for professional services and acknowledgment of the confidentiality agreement between you and your practitioner.

4. Client Premise Environment Confirmation (if applicable)
Confirm that your premises are conducive, private, safe, and suitable for session delivery. Select 'Not Applicable' for virtual or WCH facility sessions.

5. Practitioner Suitability & Availability (if applicable)
Acknowledge that client premise sessions are subject to practitioner availability and suitability assessment of the proposed location. Select 'Not Applicable' for virtual or WCH partner facility sessions.

6. Recording / Photography Consent
Indicate whether you consent to session recording or photography for training or quality purposes only.

7. Follow-Up Communication Consent
Indicate whether you consent to follow-up communication for program feedback and wellness check-ins.

Your booking is only finalised upon completion of all acknowledgments.
